Most PPO insurance plans cover outpatient mental health and addiction treatment in California. Federal parity law requires insurers to cover behavioral health comparably to medical care, and Totality Treatment Center is in-network or works out-of-network benefits with most major carriers.

Insurance carriers we work with
Select your carrier for details on what is typically covered and what you can expect to pay:
- Optum / UnitedHealthcare — does Optum cover rehab in California?
- Blue Cross Blue Shield (BCBS) — BCBS coverage for outpatient treatment
- Aetna — Aetna behavioral health benefits
- Cigna — Cigna coverage for PHP and IOP
We also work with Anthem Blue Cross, Blue Shield of California, and most other PPO plans. If your carrier is not listed, call us — we will check it directly.
Does insurance cover PHP and IOP?
Generally yes. Partial hospitalization (PHP) and intensive outpatient (IOP) are recognised levels of care with their own billing codes, and most PPO plans cover both when they are medically necessary. Coverage usually depends on:
- Medical necessity — a clinical assessment showing the level of care is appropriate
- Network status — in-network costs less, though many plans reimburse out-of-network too
- Your plan’s deductible and out-of-pocket maximum — once the out-of-pocket max is met, covered care is typically paid at 100%
- Pre-authorisation — many plans require it; we handle that on your behalf
What will I actually pay?
It depends on your plan, but the practical answer is that most clients pay significantly less than the sticker price. After your deductible, you would typically owe a copay or coinsurance per treatment day until you reach your out-of-pocket maximum. We tell you the real number before you start — not an estimate that changes later.
We also accept self-pay, and can discuss payment arrangements where insurance does not apply.
HMO, Medi-Cal and Medicare
Totality works primarily with PPO plans. We are not currently able to accept Medi-Cal or Medicare, and HMO plans usually require a referral within their own network. If your plan is not one we can work with, we will tell you honestly and point you toward providers who can help — including the free SAMHSA National Helpline at 1-800-662-4357.

Frequently asked questions
Will using insurance for treatment affect my job?
No. Treatment records are protected health information under HIPAA, and 42 CFR Part 2 gives substance use treatment records additional federal protection. Your employer does not receive clinical details.
Do I need a referral?
Not for a PPO. You can contact us directly.
What if I have a high deductible?
Outpatient care is considerably less expensive than residential treatment, and many clients meet their deductible within the first weeks of care, after which coverage improves substantially.
Can you help if you are out of network?
Often yes. Many PPO plans reimburse out-of-network care at a lower rate, and we can explain what that would mean for you before you decide.
Totality Treatment Center is an outpatient provider — PHP, IOP, and individualized outpatient programming, plus telehealth across California. We do not provide medical detox, residential treatment, or sober living housing.
